PREPARING FOR YOUR FIRST VISIT
Welcome to our practice.
As a first time patient at Monadnock Eye Physicians and Surgeons, we request that you bring the following to your appointment:
Referral, if needed
Eyeglasses, if you have them
Please review our Financial Policy Statement, with particular attention to the charge for “refraction”. A refraction is when your eyes are tested for glasses. Please note that if a refraction is done at your visit, Medicare and most other secondary insurances do NOT cover the refraction as part of an eye exam. The fee for a refraction is $27 and is due at the time of your visit.
DILATED EYE EXAM
Many patients will have dilating drops put in their eyes during their initial eye exam. This will cause blurring of vision and light sensitivity for several hours. We strongly encourage you to arrange for a driver to take you home after your appointment.
To make the time spent at your visit more efficient, we request that you log in to the Patient Portal to enter information related to your medical care and history.
TO ACCESS YOUR PATIENT PORTAL
You must request an invitation from our office to gain access to the patient portal.
You DO NOT need to have an email address to have access to the patient portal.
If you are unable to use the Patient Portal, please print and complete the Medical History Form and bring it to your appointment.
